We are looking for a dedicated Intensive Education Case Manager to join our dynamic multi-disciplinary team. In this role, you will deliver intensive case management and provide tailored transition supports to students who are at risk of disengagement, have already disengaged from education, or are at risk of involvement with the youth justice system. You will also be responsible for managing education-focused intervention and prevention initiatives, working closely with key stakeholders to create positive pathways and improve outcomes for vulnerable young people.

About The Role
Responsibilities include, but not limited to;
- Provide intensive case management and support for students at risk of disengaging from education, including education and behaviour interventions, in line with relevant laws and policies.
- Work closely with students, families, and school staff throughout the case management process, including making referrals to support services.
- Use data and information to understand each student's needs and strengths, and offer strategic advice to guide support planning and service improvements.
- Deliver best practice support by working intensively with families, schools, and community organisations to improve students' engagement and learning outcomes.
- Help create and coordinate individual support plans—such as behaviour plans, risk assessments, and transition plans—together with students, parents, and school staff.
- Ensure strategies are inclusive and culturally appropriate, particularly for First Nations students, students with disability, and those from diverse backgrounds.
- Build strong partnerships with school and department staff, as well as external agencies, to deliver positive educational outcomes for all students.
